Outpatient Care
Our outpatient services include:
- Care in a patient's home, nursing home, hospital, or at the Iowa River Hospice Home
- Our team will travel within a 50-mile radius
- Help is available 24 hours a day, seven days a week
- Hospice care to anyone who qualifies, regardless of age, illness, or ability to pay
- Grief support to family members and loved ones during and after hospice care

Admission, Referral, & Payment
Referral Process
Anyone can make a hospice referral. Call 641-753-7704. Our Referral/Admissions Nurse will contact the patient's primary physician for the required admission order. A referral visit with the patient/family will be scheduled in a timely manner.
Admission Process
The Admissions Nurse or other hospice representative, along with the patient, family, and nursing facility, will work together to complete the required paperwork. A comprehensive nursing assessment will help determine whether hospice admission is appropriate. Iowa River Hospice will then communicate with the nursing facility regarding the decision.
The Admissions Nurse reviews the Plan of Care from the nursing facility to either align interventions and goals or to work collaboratively towards optimal revision. The Admissions Nurse helps identify and secure patient supplies and any appropriate Durable Medical Equipment (DME) not already in place.
Payment Process
Hospice care at Iowa River Hospice is available to anyone who qualifies for admission, regardless of ability to pay. Medicare, Medicaid and many private insurance plans cover hospice care. If you have additional questions, please call us at any time.
